Fill-in change of
address form.

Attach the required
documents.
- Submit them

Supporting Documents
:
- A copy of the latest
assessment rate bill.
- A copy of owner’s
identity card.
- A copy of applicant’s
identity card should application be
made on behalf of the owner(s).
